How To Get Your Medical Records

Before your marijuana evaluation appointment, it’s imperative that you obtain a copy of your medical records, indicating your diagnosis. In order to get medical marijuana, you must first obtain a copy of your medical records. If you arrive to your appointment without these records, your doctor will likely be able to request the records on your behalf, but this will delay the process of obtaining the medical marijuana recommendation needed to use cannabis legally. Step 1: Contact Your Primary Care Doctor

Most practices require you to fill out a specific form to formally request your medical records. To obtain this form, simply call your primary care doctor’s office, or the office of the doctor who gave you the diagnosis, and request a copy. They should be able to deliver it to you by fax, email, postal mail or you may pick it up from the doctor’s office.

If the doctor’s office doesn’t have a specific form, simply write a letter to request your records. Be sure to include:

  • Your name, including your maiden name (if applicable)
  • Social security number
  • Date of birth
  • Address and phone number
  • Email address
  • Record(s) being requested
  • Dates you were under the doctor’s care
  • Signature
  • Delivery option (pick up, fax, email, etc.)

What if I Don’t Have a Primary Care Doctor?
