Arizona Medical Marijuana Card Evaluation Process

In  order to be considered for qualification, you must have one of the following qualifying conditions that may benefit from the therapeutic use of medical marijuana:

  • Medical Marijuana Doctor RecommendationGlaucoma
  • Human Immunodeficiency Virus (HIV)
  • Acquired Immune Deficiency Syndrome (AIDS)
  • Hepatitis C
  • Amyotrophic Lateral Sclerosis (ALS)
  • Agitation of Alzheimer’s
  • Crohn’s disease
  • Cachexia or wasting syndrome
  • Severe and chronic pain
  • Severe nausea
  • Seizures, including those characteristic of epilepsy
  • Severe or persistent muscle spasms, including those characteristic of multiple sclerosis

Step 1: Prequalification & medical Records Review

  • You do not need a referral to see a physician.
  • Complete the online Arizona medical marijuana prequalification form or contact us.
  • Your will be reviewed by our physicians and a member of the staff will call you to schedule your appointment.
  • The most important aspect of the pre-qualification process is the review of medical records to determine eligibility. Obtain your records from your treating physician(s) (see How to Obtain your Medical Records) and have them mailed or faxed to our office. (To expedite the process it is recommended that you pickup your medical records and hand carry them to your appointment)
  • If you have not seen a doctor for your condition and, thus do not have any medical records NOT A PROBLEM , please contact us to discuss your options.

Step 2: Make Your Evaluation Appointment

  • If you pre-qualify make an appointment for your doctor’s evaluation by contacting us.
  • Download the Patient Information Packet.
  • At the appointment, your physician will conduct a complete assessment of your medical condition along with any pertinent exams. As a result of this assessment, you will receive a detailed treatment plan along with a signed Physician’s Certification Form.

You must bring the following with you to your appointment.

  • Complete and signed Patient Information Packet.
  • Arizona drivers license or state issued ID to establish proof of residency.
  • Medical records (if not previously mailed/faxed)
